If you already have an account on Czech Y-DNA, click HERE and enter your User ID and Password to create a joint account for both databases.
Here create your database record.
In any case you met difficulties, ambiguity, please send a mail to cmtdna@genebaze.cz.

Filling procedure:
- Select mutations locations and set theirs values.
- Select laboratory.
- Insert name of contact person responsible for your data in our database.
- Insert surname of most distant known maternal ancestor on the direct female line.
- Choose place where she lived or write it, if not on the list.
- Enter contact e-mail (2x).
- Enter your password (2x).

In the case of success you will see a page with your new ID and password. You will use them for your database entry.
In the same time you will get an e-mail with your ID and password.
